Diabetes Québec created the Support Program for People living with Diabetes. This program is being offered in partnership with the Ministère de la Santé et des Services sociaux du Québec (MSSS).

The program offers support through distance care and education to two types of individuals with diabetes:

  1. Individuals with type 2 diabetes without a family doctor;
  2. Individuals with type 1 diabetes without an endocrinologist.

If you are living with type 2 diabetes

To participate in the program, you must:

  • Be at least 18 years of age;
  • Have a valid RAMQ number;
  • Have been diagnosed with type 2 diabetes;
  • Have no major diabetes complication (for example, undergoing dialysis);
  • Not have a family doctor.

If you meet these criteria, you will be able to benefit from distance (virtual) care. This care includes a meeting for an initial assessment, health tests (if needed), access to educational capsules on type 2 diabetes and a minimum of four meetings with a multidisciplinary team specializing in diabetes. The program lasts from four to six months.

This program does not replace the care and monitoring provided by a family doctor. Instead, the program makes it possible to assess the state of health of people living with type 2 diabetes, offer them basic education to help them better manage their disease and support them in their efforts while they wait to be taken on by a family doctor.

How to participate

To receive care, you must complete the registration form. The registration form is in french only. If you are unable to complete it, please contact us by telephone (see our contact details below).

  • Have a valid RAMQ number;
  • Have been diagnosed with type 1 diabetes or LADA;
  • Have a request from your family doctor or a specialist nurse practitioner;
  • Not be under the care of an endocrinologist.

If you meet these criteria, you will be able to benefit from distance (virtual) care. An innovative and essential program

Self-management and multidisciplinary support are the keystones of diabetes treatment. Unfortunately, Quebecers with diabetes have unequal access to the education and care they need. Although it has been proven that early management and education can improve quality of life and reduce the risk of long-term complications, services have been cut in recent years while the number of diabetes cases continue to climb. In light of this situation, Diabetes Québec is proud to offer this program to the population with diabetes and without proper access to care!
