This program is being offered in partnership with the Minist\u00e8re de la Sant\u00e9 et des Services sociaux du Qu\u00e9bec (MSSS).<\/strong><\/p>\n\n\n\n<p>The program offers support through distance care and education to two types of individuals with diabetes:<\/p>\n\n\n\n<ol class=\"wp-block-list\" type=\"1\">\n<li><strong>Individuals with type 2 diabetes without a family doctor<\/strong>;<\/li>\n\n\n\n<li><strong>Individuals with type 1 diabetes without an endocrinologist<\/strong>.<\/li>\n<\/ol>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:33.33%\">\n<figure class=\"wp-block-image size-large is-resized\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"369\" src=\"https:\/\/www.diabete.qc.ca\/wp-content\/uploads\/2024\/01\/EN_Logo-Programme-Diabete-Full_RGB-1024x369.png\" alt=\"\" class=\"wp-image-32797\" style=\"aspect-ratio:2.7750677506775068;width:422px;height:auto\" srcset=\"https:\/\/www.diabete.qc.ca\/wp-content\/uploads\/2024\/01\/EN_Logo-Programme-Diabete-Full_RGB-1024x369.png 1024w, https:\/\/www.diabete.qc.ca\/wp-content\/uploads\/2024\/01\/EN_Logo-Programme-Diabete-Full_RGB-300x108.png 300w, https:\/\/www.diabete.qc.ca\/wp-content\/uploads\/2024\/01\/EN_Logo-Programme-Diabete-Full_RGB-768x277.png 768w, https:\/\/www.diabete.qc.ca\/wp-content\/uploads\/2024\/01\/EN_Logo-Programme-Diabete-Full_RGB.png 1318w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n<\/div>\n<\/div>\n\n\n\n<h3 class=\"wp-block-heading\">If you are living with type 2 diabetes<\/h3>\n\n\n\n<p>To participate in the program, you must:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Be at least <strong>18 years of age<\/strong>;<\/li>\n\n\n\n<li>Have a <strong>valid RAMQ <\/strong>number;<\/li>\n\n\n\n<li>Have been <strong>diagnosed with type 2 diabetes<\/strong>;<\/li>\n\n\n\n<li>Have <strong>no major diabetes complication<\/strong> (for example, undergoing dialysis);<\/li>\n\n\n\n<li><strong>Not have a family doctor<\/strong>. <\/li>\n<\/ul>\n\n\n\n<p>If you meet these criteria, you will be able to benefit from distance (virtual) care. This care includes a meeting for an initial assessment, health tests (if needed), access to educational capsules on type 2 diabetes and a minimum of four meetings with a multidisciplinary team specializing in diabetes. The program lasts from four to six months.<\/p>\n\n\n\n<p><strong>This program does not replace the care and monitoring provided by a family doctor<\/strong>. Instead, the program makes it possible to assess the state of health of people living with type 2 diabetes, offer them basic education to help them better manage their disease and support them in their efforts while they wait to be taken on by a family doctor.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">How to participate<\/h4>\n\n\n\n<p>To receive care, you must complete the registration form. The registration form is in french only. If you are unable to complete it, please contact us by telephone (see our contact details below).<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link wp-element-button\" href=\"https:\/\/www.diabete.qc.ca\/le-diabete\/la-vie-avec-le-diabete\/participer-au-programme\/\">Access the registration form<\/a><\/div>\n<\/div>\n\n\n\n<h3 class=\"wp-block-heading\">If you are living with type 1 diabetes<\/h3>\n\n\n\n<p>To participate in the program, you must: <\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Be at least <strong>18 years of age<\/strong>;<\/li>\n\n\n\n<li>Have a <strong>valid RAMQ <\/strong>number;<\/li>\n\n\n\n<li>Have been <strong>diagnosed with type 1 diabetes or LADA<\/strong>;<\/li>\n\n\n\n<li>Have a<strong> request from your family doctor or a specialist nurse practitioner<\/strong>;<\/li>\n\n\n\n<li><strong>Not be under the care of an endocrinologist<\/strong>. <\/li>\n<\/ul>\n\n\n\n<p>If you meet these criteria, you will be able to benefit from distance (virtual) care. This care includes an initial assessment with a clinical nurse specializing in diabetes, health tests (if needed), access to the <a href=\"https:\/\/type1support.ca\/en\/landing\/\" target=\"_blank\" rel=\"noopener\">BETTER project\u2019s online training and educational platform<\/a> and a minimum of two annual meetings with an endocrinologist.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">How to participate<\/h4>\n\n\n\n<p>To participate, you, your doctor or a specialist nurse practitioner<strong> <\/strong>must send us a request. This request can be sent by email to programme@diabete.qc.ca or by fax to 514 259-9286.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">An innovative and essential program<\/h3>\n\n\n\n<p>Self-management and multidisciplinary support are the keystones of diabetes treatment. Unfortunately, Quebecers with diabetes have unequal access to the education and care they need. Although it has been proven that early management and education can improve quality of life and reduce the risk of long-term complications, services have been cut in recent years while the number of diabetes cases continue to climb.
